MyChart Services (through Allina) is available to assist you 7-days a week, 7am-9pm toll free at 1-855-551-6555. Otherwise the Release of Information office at Western Wisconsin Health can assist you Monday through Friday 7am-4:30pm, 715-684-1590.

What to do if you forgot your MyChart ID?

If you forget your MyChart ID, please call MyChart Services toll free at 1-855-551-6555. You will be given a new access code to sign in to MyChart. Once you have entered your access code, Social Security number and date of birth, your original MyChart ID will appear in the MyChart ID field.

How to cancel a clinic appointment?

To cancel a clinic appointment, click on “Appointments” and choose “Cancel Appts.”. If a button does not appear to the left of an upcoming appointment, you will not be able to cancel that appointment using MyChart. Please call the clinic to cancel this appointment.

MyChart access is permitted only to authorized users that have been verified through an activation process. MyChart members have a unique MyChart ID and password that is known only to them.
