Can I Email my health forms to WPI?

Mar 02, 2022 · WPI Health Services is an appointment-based student health center. Please call our office at 508-831-5520 to schedule an appointment. We are unable to accept walk-in appointments at this time and due to the high volume of patient appointments, we will be unable to see students who are arriving late for their scheduled appointments.

Vaccine Exemptions

WPI provides a process for students to submit a request for a medical or religious exemption for all required vaccinations. Students must complete a Vaccine Exemption Form and specify whether they are asking for a medical or religious exemption.
