Referrals are given on a rotating basis, and you are responsible for identifying and selecting a physician who meets your particular health needs and preferences.

Is West Park Hospital registration free?

Registration is quick, easy, and free. You begin your registration process at our West Park Hospital campus—be sure to provide a personal email address to use for the account at Registration/Patient Access. From there, patients can self-enroll online or stop at Health Information Management to create the account.

Please download and print this form to request access to another’s records, for example a minor child age 0-13, an adolescent age 14-17, an adult 18+ or as a legal representative. Fill out and return to the Wayne Memorial Medical Records Department.

How to create a health care account?

To create an account for yourself, register online or ask your health care provider to email you an invitation during your next visit. If you register online, you can choose to receive an access code via email or text message. That code will be used to create your new account.

Can a 13 year old register for my health records?

Minor patients ages 13-17 may register individually for access to the My Health Records Patient Portal, unless determined by their physician, that the minor patient is not competent. Absent such a determination, the minor patient will control his or her own medical information.
