Urgent Care, After Hours and Emergencies

After office hours, please call 774-462-3910 and the physician on call will call you back after you leave a message. In a life-threatening emergency, please call 911 immediately.

Medical Records

If you need a copy of your medical record, please download our Medical Record Release Form . Print, complete, and return it to Milford Regional Physician Group via mail or fax as indicated on the form.

Insurance

We are providers for most medical insurance plans. Because there are a wide variety of insurance options, please contact your insurance company directly to make sure your doctor accepts your plan. We provide equal access to our patients regardless of the source of payment.

Make an Appointment

Patients are seen by appointment – on the same day the appointment is made if possible. To schedule an appointment, please call $host.phoneNumber during regular business hours.

Telephone Calls

All patients are encouraged to call during office hours. If our nurse cannot answer your question, she will confer with the doctor. All calls will be returned promptly. Please confine all requests for prescription refills to office hours whenever possible. If you have any emergency after hours call our office.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Billing

Unless arrangements have been made in advance, co-payments, co-insurance, and any outstanding balances are expected at the time of service. Patients may be financially responsible for payment of all services even if their insurance company does not pay.

Insurance

We participate with most major insurance plans, including Medicare; however, please review all insurance information with our staff prior to services being rendered.

Cancellations or Rescheduling

If you are unable to keep your appointment, please notify us at least 24 hours in advance. Missed appointments will result in a $25.00 dollars fee added to your account. Please call and give us a chance to offer help with your scheduling problem.

Refill Requests
