Alliance Women’s Healthcare uses Aprima Patient Portal, a password-protected and secure section of our site that is designed with your privacy in mind. The patient portal allows you to update demographics, add or update pharmacy information and view billing statements.

Before your appointment, please log in to the patient portal and fill out your patient paperwork prior to your appointment. (If you are having trouble logging into your patient portal, call 817-741-9663 during normal business hours, and we’ll get your password reset.)
New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms with you to your appointment.
