Proxy Access to myWMH patient portal accounts

Please download and print this form to request access to another’s records, for example a minor child age 0-13, an adolescent age 14-17, an adult 18+ or as a legal representative. Fill out and return to the Wayne Memorial Medical Records Department.

Introduction (Step 1 of 6)

You must fill out an authorization form and submit with a Photo I.D. Please note that your provider sends you your patient code via email or letter, depending on what you requested. If you do not have a patient code, please contact your provider directly.

Login Information (Step 2 of 6)

Username: Create a username between 4 and 32 characters long. No other user can have the same username.

Security Questions (Step 3 of 6)

Set your own security questions. These will be used if you ever forget your password. Security Question: Click the drop-down arrow and select a question, you can select any security question from the list. At the Your Answer: prompt enter your response, make sure your response is memorable but hard for others to guess.

Link Account to Secret Code from your Provider (Step 4 of 6)

First Name: Enter your full first name as your provider entered it on the patient code document you received. Typically this is consistent with your name as it is spelled on your ID card.

Release Consent (Step 5 of 6)

Your provider requires that you consent to the release of medical records to you via the Patient Portal. You must click in the check box next to the prompt: ‘I consent to the release of the patient medical records stored here as stated above.’

Review Registration Information (Step 6 of 6)

Below is the information you specified. Please review it for accuracy. If the information was entered correctly please hit REGISTER; if you would like to edit an entry hit the BACK button to the page associated with the entry you would like to edit. If you would like to end the registration without filing please hit the CANCEL button.
