What is Willamette Valley's patient portal?

Current Patients. Please arrive 10 minutes prior to your scheduled appointment time for check-in and preparatory paperwork. If you arrive more than 10 minutes later than your appointment we may not be able to see you as there will be other patients that need to be seen and adequate time must be given to administer proper patient care.

How do I log in to the patient portal?

Willamette Valley Medical Center's patient portal is an online service through which hospital inpatients can review their personal care summary, record their own personal health notes, and list their medications and allergies. Your username will always be your email address. Each time you enter your Portal account, you also will need to enter your password and answer your security question for extra security.

When you sign-up for Health Record Home, you will be able to: 1 View and share your personal health record (PHR) related to your hospital stays. 2 Have electronic access to an updated list of medications, diagnoses, allergies, lab test results, patient history and other healthcare information related to your inpatient hospital visit. 3 Access your hospital discharge instructions for better after-discharge care. 4 Manage family members’ health records once you are authorized.
