This interactive web portal empowers all Community Hospital patients and their families to take a more active role in their care by providing easy, secure access to health information and online communication with Patient Portal staff. The new portal interface is named MyCareCorner. On your first visit to the hospital or clinic you will receive an email with a link and instructions on how to set up your patient portal. If you previously used our Thrive Patient Portal, please call or email to request a MyCareCorner (MCC) link to set up your account.
For questions regarding the Patient Portal, please call (970) 644-4002. Parents/guardians can request access to their minor child’s MyHealth Portal Portal, for children under 12 years of age, by clicking on the “I do not have an account” button above. Please provide the information for the child and sign as the parent/guardian.
