If you are over the age of 18 and your email address has previously been provided to us, and is stored in our system, or if you have a medical record number (MRN) from our new EMR system (after 10/01/2018).

For patients at St. Luke’s Health–Brazosport Hospital, we offer a separate, secure portal that allows you to access your health information, update your personal information, check account balances, and view scheduled appointments all in one place.
For patients at St. Luke’s Health–Patients Medical Center, we offer access to MEDITECH. Log in to your account to find your medical records, schedule a visit, and more.
For patients at St. Luke’s Health–Memorial, we offer access to MY ONECARE. Log in to your account to find your medical records, schedule a visit, and more.
You can use another application to access your health information, if you prefer. To do so, you will need to first set up a patient portal account. The username and password you choose will be the security component for your health information and make it possible for you to access and import it.

Proxy access allows parents, legal guardians or caregivers to access a patient’s health information on the patient portal.
You will need to fill out forms for each child you would like to enroll into the portal.
In order to be in compliance with state of Montana law concerning certain medical/mental health records of minor patients, patient portal proxy access will not be provided to adolescents between the ages of 13 – 17.
An adult patient or their legal representative may remove proxy access for any reason and at any time. You can call St. Luke’s Health Information Management department at 406-528-5268 for information about how to revoke proxy access.
There is no guest option for the MyHealth Patient Portal. Any user that has access to a patient’s record would need to have access as a proxy user.
