Westchester Health will access and use your information on the Patient Portal in accordance with the Westchester Health Patient Portal Consent Form which you must complete in order to access the portal and Westchester Health’s Notice of Privacy Practices . As a summary, Westchester Health may use your information in order to facilitate scheduling, billing, and your treatment, as well as to communicate with you about your care.
Patient Portal. This portal is for patients of Williamson Medical Center. This would include visits to our emergency room, surgery, radiology and laboratory. For questions regarding the Williamson Medical Center patient portal, contact WMC’s Medical Records Department at 615-435-5757.
Please note that parents can obtain proxy access to their child’s records through the portal from birth to age 13. At age 14 proxy access to the child’s records through the portal will be suspended. To obtain records for minor children between the ages of 14 and 17 please complete a request form or contact the Health Information Management Department at 615-435-5750 for more information or to set up proxy access for a minor child under 14.
The Patient Portal is a secure, confidential and easy to use website that gives patients 24 hour access to their medical records. It uses the latest encryption technology to deliver secure communication between patients and providers. The Patient Portal is administered and maintained by NextGen Healthcare Information Systems, LLC (“NextGen”) on behalf of Westchester Health.

Fill out the My LifeBridge Health online request form, which can be downloaded here. You will receive an email with a link and instructions to complete your registration.
We have made it easy for patients to review their test results through our online LifeBridge Patient Portal delivering results of these tests as quickly as possible (six days after the test was completed). The CCMH lab has the capability to do many lab tests that are ordered by your physician. All lab work that is performed here at our facility will show in your patient portal.
