To request a Virtual Visit appointment (i.e., Telehealth), call our office: 413-562-5173 and request an appointment. Once you have registered with and have access to the new Patient Portal, you will then be able to request any type of appointment from the patient portal! New Patients

We are still welcoming new patients. Our goal is to provide each patient with a positive patient experience as we deliver quality healthcare services. Our staff will collaborate with you to develop a personalized plan to manage your care. Here are some important things you should know before booking your first appointment:

IDENTIFICATION AND INSURANCE CARD (S)

Please bring your ID and insurance card (s) to your visits so we can be sure we are billing for the services properly.

MISSED APPOINTMENTS

It is very important that you attend your scheduled visits. Our office has a high demand for new patients. Please be aware that if you miss your first appointment, we will not likely register you as a new patient and you will have to seek care elsewhere. If you are unable to keep your appointment, at least 24 hours notice is required to reschedule.

MEDICATION BOTTLES

Your safety is of great concern to us. An accurate, up-to-date medication list is very important for your care. Please bring all your medication bottles to your visit.

PATIENT PORTAL

You will be asked by our staff if you are interested in our secure online Patient Portal where you can request appointments, see your lab results, review your health history, and communicate with your primary care physician team. We are getting positive feedback from many patients about this free service.

APPROPRIATE HEALTHCARE SETTING

Your primary care physician and their care team play an important role in the coordination of your care. We are often in the best position to help you identify the most appropriate level of care, and refer you to specialty care providers that share our patient-centered approach.

NEW PATIENT INTAKE FORM

We ask that each new patient complete this intake form on or before their first visit.

Ellen has been my primary care provider for 25 years. I am a registered nurse. I trust her care, her professional attitude and her medical opinion.
