May 03, 2016 · Patients who wish to view their patient portal account can visit www.wesleymc.com/patient-portal. New users will be directed to a page that informs the patient that Wesley has upgraded its portal and lets them know they must create a new account with My Health One. Information about the portal is available at all Wesley facilities and at discharge.

Merit Health Wesley provides CareNotify ™, a system to help you be informed about your care and what you need to do to stay healthy. CareNotify ™ includes a Patient Portal as well as capabilities for direct communication to patients via text or email.

While we strive to keep all of the information in your records complete and accurate, if you identify any error or discrepancy you agree to notify us immediately. All new and established patients have signed our HIPPA agreement form and have been given a copy of our Notice of Privacy Practices. Secure messaging can be a valuable communications tool, but has certain risks. In order to manage these risks we need to impose some conditions of participation. By signing our Consent Form you accept the risks and agree to the conditions of participation. Please keep all portal access, user information, and instructions in a secure place. Once logged into the portal, you can go to “Edit Account” on the top left of the page to change your User ID or Password.
