Charges fall into two categories: a basic daily rate, which includes your room, meals, nursing care, housekeeping, telephone and television and charges for special services, which include items your physician orders for you, such as X-rays, laboratory tests and surgical services. The per page charge is as follows: 1 Electronic records delivered in electronic format—$6.50 2 Medical record with paper records delivered in electronic format are billed at $6.50 + 7¢ per page labor cost to create and deliver the portion of the record maintained in paper. 3 Electronic records delivered in paper 90¢ labor cost to create and deliver the portion of the record maintained electronically plus 5¢ per page for paper and toner. 4 Paper records delivered in electronic format 7¢ per page labor fee. 5 Paper records delivered in paper 12¢ per page.
