How do I obtain patient information from WellStar hospital?

This can be mailed to Health Information Management, Release of Information Services, at the address of the appropriate Wellstar Hospital. On your request letter and patient authorization identify the hospital where patient was treated. Fax requests are only accepted from healthcare providers and patients.

How to obtain a copy of a Wellstar medical record?

To obtain a copy of a Wellstar medical record, we require a written request and properly executed patient authorization for disclosure of protected health information. This can be mailed to Health Information Management, Release of Information Services, at the address of the appropriate Wellstar Hospital.

If you seek hospital records or records from any other medical facility, you will want to request them directly from that facility.

How to get a medical record?

If you wish to obtain medical records for you personal use we are happy to assist you in this process. The per page charge is as follows: 1 Electronic records delivered in electronic format—$6.50 2 Medical record with paper records delivered in electronic format are billed at $6.50 + 7¢ per page labor cost to create and deliver the portion of the record maintained in paper. 3 Electronic records delivered in paper 90¢ labor cost to create and deliver the portion of the record maintained electronically plus 5¢ per page for paper and toner. 4 Paper records delivered in electronic format 7¢ per page labor fee. 5 Paper records delivered in paper 12¢ per page.

Who can access medical records?

To request medical records, you must be the patient, or the parent or guardian of the patient for whom you seek records. Caregivers may be able to access records if the patient has provided written permission to the provider.

Can you request hospital records from another hospital?

If you seek hospital records or records from any other medical facility, you will want to request them directly from that facility. Be aware that you may be denied some records, usually related to mental health records.

Can a provider deny access to medical records?

If a provider believes that letting you look at your medical records can endanger your physical health, your request may be refused. They cannot deny you access just because they think you will be upset, unless they believe revealing the information might lead you to physically harming yourself.

Who must sign the consent release?

The parent or legal guardian must sign the consent release. Legal guardians will be asked to provide documentation of legal guardianship.

How long does it take to get a MyChart account?

We need some information from you in order to grant you a MyChart account. Once submitted, it could take up to 48 hours for processing and verification. Once approved, you will receive an email or a letter with your activation code and instructions on how to activate your MyChart account.
