The health information in your Connect account comes directly from your doctor's electronic medical record. For outpatient records, you can ask your doctor to correct any inaccurate information at your next visit, or send a message through Connect to your provider’s office to request corrections to your health record.
Any adult can be given access to another adult’s account, by granting portal proxy access. The portal proxy does not need to be a patient. The patient will need to complete the Connect Patient Portal Proxy Access Authorization form.
