How to send a message on a portal?

From the main page of the portal site the very first tab is Messages. To create a new message click the New Message option at the top. You will then see a screen similar to the to the picture above . You can select the provider you see, if you are unsure of which provider you see any provider will do, your message will be addressed and sent on to the appropriate provider. Then you can select the reason that is most appropriate to your situation. Write a subject in the Subject line and then write your message in the Message box. Please note any urgent issues should be called into the office. Your portal message will be addressed within a 24 to 48 hour time period.

How to get a copy of my child's physical?

To download a copy of the physical form you received at your child’s physical from the portal you must request one be sent to you . It will appear in the message area of your portal and you can then print out the health appraisal at home.
