How does the webchart system work?

The Webchart system also provides for a way for our patients to securely access their health records from the convenience of their own computer by way of a “Patient Portal”. The Patient Portal allows our patients to: Login to the patient portal to view scheduled appointments or schedule a new one.

With MyChart, patients now have access to their health information 24/7 via the MyChart website or mobile app. Complete PreCheck-In registration information up to 5 days before an appointment View labs and test results in rapid time, including COVID-19 results

With the MMG Patient Portal, Patients are able to request prescription re-fills online. Log on to the Patient Portal to view your health records in the privacy of your own home.

How do I Activate my MyChart account?

If you have not activated your MyChart account yet, you can do so by going to the MyChart log in page and following instructions to set up an account. With MyChart, patients now have access to their health information 24/7 via the MyChart website or mobile app.

What is FollowMyHealth portal?

The FollowMyHealth portal is intended to provide users with their health information, educational materials, and communication tools. It is not intended to provide medical advice. If you have any need for medical or personal advice or have any questions regarding your health, please consult your physician. DO NOT use FollowMyHealth portal for urgent medical matters. If you are experiencing an urgent medical problem, call 911 or your physician’s office immediately.

What is follow my health?

Through the FollowMyHealth patient portal, registered users can request online access to medical information for an adult in their care who has impaired decision-making abilities. Proxy access enables you to view the adult patient’s health information in the portal and, in some instances, communicate through the portal with the adult patient’s healthcare team.

Does Northwell Health have a follow my health app?

Northwell Health patients will encounter the FollowMyHealth name when receiving emails about the FollowMyHealth website. Patients registered for the FollowMyHealth patient portal can also download and use the FollowMyHealth app for iPhone/iPad and Android.
