Patients who take critical prescription medications are advised to adhere to the schedule of lab and office visits recommended by their doctor.

Medical Records

A medical request signed by the patient is required to send or release records to anyone. If you are requesting a copy of your records, there may be an additional fee.

We accept most major insurances. You should have your insurance card available at all appointments. Copays, deductibles and/or co-insurance must be paid at time of service. We accept cash, checks and credit/debit cards (Visa, MasterCard and Discover).
