If you are a patient of Warner Hospital & Health Services, you can access the patient portal, My Health. This online tool gives you access to your health information. My Health is available over the internet which means you can use it from virtually anywhere at any time.

How do I access the patient's record?

The patient's record will be accessed through your MyChart record. As of April, 2021, patients will have quicker and broader access to their medical record and provider notes. As part of this initiative, you will get lab and imaging reports at the same time as your provider.

How do I sign up for portal enrollment?

You can receive the sign up packet at Patient Registration or the Medical Records Department.  Or complete the form, Patient Portal Enrollment Request, found at the bottom of this page and email back to himquality@warnerhospital.org.

What to do if patient care is not addressed?

If an individual has any concerns about patient care and safety in the hospital that the hospital has not addressed, patients are encouraged to contact hospital management.
