What is the patient portal for children's clinics?

We want to remind everyone about the importance of social distancing and face coverings while in public and to be aware of community-specific mandates for wearing a mask or face covering. As the number of COVID-19 cases continues to grow in our communities, we want to offer a few reminders for patients WHO TEST POSITIVE OR ARE EXPOSED TO COVID-19.

Our Clinics Remain Open

We are all aware of the public health concerns due to COVID-19, and our doctors, nurses, and staff remain committed to serving you and your family. We have updated some of our clinic procedures so we can continue to provide you with excellent care.

Telehealth Services

We are temporarily offering telehealth services for certain types of appointments. Call our clinics to find out if a televisit might be appropriate for your child.

Keeping You and Your Family Safe

Please continue to attend your regularly scheduled well visits. Well visits are essential to ensuring the health and wellbeing of your children. We are conducting well visits and non-sick appointments in the mornings and are seeing sick patients later in the day to help in eliminating cross contact between sick and healthy patients.

Sports Physical Packet

Below you will find the sports physical packet required by TSSAA in order for your child to participate in school sports. The sports physical packets are labeled by county. Please fill out and sign the History Form and fill out the top portion of the Physical Examination form. Bring these forms with you to your office visit.

Insurance

While we participate in most insurance plans, there are a few we do not. Please call your insurance company to confirm we are participating with your plan. As a courtesy, we will automatically file claims to all policies we participate with.

Fees and Payment

Co-pays, co-insurance, and deductibles are expected at time of service. This is a requirement of your insurance.

Billing Office Hours

Our billing office is open Monday thru Friday from 8:00 am to 4:30 pm. Please do not hesitate to call with any questions. Most questions can be answered by our billing office without having to call your insurance.

Other Information

Please be aware of your insurance benefits prior to your visit. Our office offers services, procedures, and vaccines that may/may not be covered by your plan. There is no way for us to know benefits for each patient due to the various plans each carrier offers.
