Our patient navigators streamline your care within the Vanderbilt Health system and ensure that you have all the information you need for successful participation within the MyHealth Bundles program.
Educators. Patient navigators begin by conducting phone consultations with prospective patients to educate patients on the bundle they’re interested in and answer questions. Navigators then conduct eligibility screenings and begin the enrollment process if the patient qualifies. The navigators also stay up to date on changes in each bundle and clarify those revisions for patients so that they’re clearly understood.
Please note: The MyHealth Bundles office is closed and our patient navigators are unavailable during the following holidays: New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Eve and Christmas Day.
