A Rural Health Clinic (RHC) can be a public, nonprofit, or for-profit healthcare facility with two main requirements needing to be met in order to be considered for RHC status.
Van Diest Family Health Clinics in Webster City, Jewell and Stratford provide primary care services for Medicare and Medicaid patients, are located in provider shortage areas, and already utilize a team-based approach with physician and non-physician collaboration to provide services.
For the majority of patients visiting Van Diest Family Health Clinics, there will not be a noticeable difference in the care received since receiving accreditation. However, there are a few changes with respect to specific medications being administered that affect some patients and where they will now receive these medications.

Medical records will not be released without a written authorization. For continued patient care directly to a physician’s office or healthcare facility or in the event of an emergency, Van Diest Medical Center may also request written authorization by the patient or responsible physician.
NextGen Clinic Patient Portal is now available at several MercyOne Clinics. The NextGen Patient Portal allows you to connect with your doctor or primary care provider at a time that works best for you. Request an appointment, communicate with your doctor’s office and get health information online.
If you have not accessed your portal since prior to June 2019, you will need to create a new account.
