How do I use the student health portal?

Sign into the Patient Portal (use your UW user name and password) and enter your MMR dates or titer results under the MMR tab. a. Enter other immunizations and dates under the Recommended tab. b. Hit the Submit key when you have entered all your immunization information. c. Upload the documentation of your immunizations using the “Upload” tab.

My Patient Portal

Patient portals are interactive health records giving you convenient, 24-hour access to your personal health information. All you need is an internet connection.

Do I have to have a separate login for each of my children?

You do NOT have to have a separate email for each child as long as you use the same email address to set up each account. When you log into the Portal, you can toggle between your personal portal and their portal accounts. Please note, when your children turn 18, this link will break and they will need to create their own account.
