UW Health MyChart is a secure online service that allows you to view portions of your UW Health medical record, receive test results, schedule simple appointments, and more.
e-Visits are handled by a UW Health physician assistant, nurse practitioner or physician who works in a UW Health urgent care or primary care clinic during the following hours:
Log into your MyChart and type in “request proxy access” in the search menu bar. Select “other account” to click the “request access” button. Select the appropriate proxy relationship and follow the instructions.
Sign up at mychart.uwhealth.org: Instantly activate your account by going to mychart.uwhealth.org and selecting Sign Up Now, then Sign Up Online, then 3rd Party Identity Validation. Follow the process to validate your identity. After the validation process is complete, you will be able to access your MyChart account. You may also choose U.S. Mail if you would like an activation letter mailed to your home.

Having a patient portal account helps the practice create a more accurate record of your medical history as well as educate you on certain aspects of your health. It provides you and the practice with a more efficient means of communication, eliminating sources of frustration such as phone tag.
What do I need to use MEDENT Patient Portal. You will need access to a computer connected to the internet and an up-to-date browser (such as Internet Explorer or Safari). You will also need an email address. The email address you provide is only used to notify you when you have a new message in your portal account.
