This system was renamed and upgraded in November 2019, as part of a UVM Health Network-wide transition to a more connected, safe, secure and convenient online health experience for all patients. This upgrade occured at all UVM Medical Center practices and locations, with the exception of Dialysis.
Porter Medical Center’s Patient Portals are online tools that help you to manage your healthcare easily and securely, allowing you to view your personal health record whenever and where ever you have access to the intranet. Porter has two separate patient portals that you can access, depending on the service you are using:
Important message for patients of Central Vermont Medical Center, Porter Medical Center, and Champlain Valley Physicians Hospital. As of November 9, 2019, these hospitals' outpatient portals have been replaced by MyChart, a new and improved patient portal powered by Epic.
The FIRST STEP is to sign up for the Portal at your local provider office or Porter Hospital. (Due to HIPAA requirements, you must sign up in person) Once the Portal is activated, you will log on using the the “Hospital Portal” link above. Then, simply enter your user name and password.
Visit MyChart.UVMHealth.org and click Sign up now. You can also skip this step by requesting an account activation code from your medical provider at your next visit or using the code at the bottom of your printed After Visit Summary.

UVM Health Network – Porter Medical Center, located in Middlebury, Vermont, is comprised of a 25-bed acute care facility, a 105 bed skilled nursing facility which includes a memory care unit, short-term rehabilitation unit, and a long-term care unit. Porter operates a network of primary care and specialty medical practices throughout Addison County and the town of Brandon, as well as an infusion center and starting this summer “ Porter ExpressCare “.

Under the affiliation, the UVM Medical Center , Alice Hyde Medical Center , Central Vermont Medical Center , Champlain Valley Physicians Hospital, Elizabethtown Community Hospital, and Porter Medical Center will remain separate free-standing hospitals with their own boards, management, workforce, licensure, medical staff (including credentialing and governance) and endowment. The medical staffs of each hospital will remain separate with separate credentialing and governance.

As of November 9, 2019, you should stop using your existing MyHealth patient portal to access information from outpatient practices and clinics. You will still have access to your information in this portal after November 9, 2019, but no new outpatient information will be added. To view new outpatient medical information and test results, see upcoming appointments, and send messages, you should create MyChart accounts at MyChart.UVMHealth.org.
