Here you can view your latest health care information, request appointments, and engage with your healthcare team. You must first Sign up for a MyMoffitt Patient Portal account by visiting https://my.moffitt.org.

Log In > CREATE YOUR ACCOUNT If you’d like to create a new account or if you created an account on or before June 22, 2018, please enroll here. You will need your Medical Record Number and an e-mail address to create an account.
Sign In Sign In to MyMoffitt Before Sign In An email address will be required to sign into the MyMoffitt patient portal. If you do not have an e-mail address, you can get one free of charge at these websites – Google, Yahoo or Microsoft Outlook. You will need access to a high-speed internet connection.
So very useful and intuitive to use. My only suggestion would be to add a simplified version with less data and buttons linked to most commonly used resources, I.e. messages, appointments, lab results. However this does not detract from my 5 star rating!
It has all the information about you in this app including the times and dates of your appointments and the location of them. Great app.

You must be a registered patient with Moffitt Cancer Center and have your Moffitt Medical Record Number. Access to an email account is required. You can sign up for a free email account at a number of websites including www.gmail.com, www.outlook.com, or www.ymail.com.

Patients can request copies of their medical records by completing an Authorization for Use or Disclosure of Protected Health Information form electronically through the MyMoffitt Patient Portal, by downloading and completing the form from Moffitt.org, or making a request in person.
CernerHealth and IQHealth are systems from Cerner used to provide secure access and securely manage your electronic medical record.
The Health Portal can be used to make appointments, view labs, and view immunizations. We encourage our patients to use this whenever possible to save them time and provide them access to their labs and immunizations outside of operating hours.
Labs can be checked online via the health portal. Access the portal and choose the View Labs tab.
