Medicare-eligible beneficiaries entitled to Medicare based on age can no longer enroll in the US Family Health Plan. If you enrolled in the US Family Health Plan on October 1, 2012 or later and you are under age 65 and entitled to Medicare, you may remain enrolled in the US Family Health Plan until your 65th birthday.
If you are under age 65 and using Medicare due to a disability or other reason, you can remain enrolled in the US Family Health Plan until you age out. At that time, you will transition to TRICARE For Life.

You'll get all care (including prescription drug coverage) from a primary care provider that you select from the network of private physicians affiliated with one of the not-for-profit health care systems listed above.
