MyChart, the UR Medicine online patient portal MyChart, the UR Medicine online patient portal, provides you a way to more actively manage your health care. Whether you’re maintaining good health or managing a chronic condition, we want to make sure you have your most up-to-date medical information available to you.
Mar 04, 2022 · Visits with providers at all UR Medicine-affiliated practices and facilities, as well as results and bills will be available through UR Medicine’s MyChart portal. Please consider registering for MyChart to begin using its many online services, from appointment scheduling, provider messaging, online bill-pay and more.

To sign up for MyChart, you will need an activation code. This code allows you to log on and create your new MyChart user ID and password, which you will then use to log on to MyChart. All patients over age 18 will receive an activation code at the end of each UR Medicine visit, so you can sign up at home when it’s convenient for you.

1. As we provide your health care, we are required to maintain documentation of your medical history, current conditions, treatment plan and all treatments given, including results of all test, procedures and therapies. NOT ALL of your Hospital medical record may appear on your MyChart.
