To ensure your visit to our office is as convenient and efficient as possible, we are pleased to offer our patient forms on-line. These forms may be printed, filled out manually and brought with you to your first visit. Before sending in these forms, please note we are an LGBTQ focused practice.

Get Results of Labs or Other Tests

All lab tests must be ordered by a physician, thus you will need an appointment with your provider to get any lab test ordered. Results will be sent to your portal, and you can view them online. They can take anywhere from 2-7 days depending on the test being ordered (some even longer).

Request Refills of Medications

Refills on medications should only be done during your appointment with your provider. Please check your refills before you come see us, and confirm you will have enough medication to last until your NEXT appointment.

Request a New Medication

Any new medication prescription will require an office visit with a physician. Please call the office to inquire.

Are you sick? Want an Antibiotic?

In general, our physicians do not prescribe antibiotics without an office visit. Most respiratory infections are self-limited and will not be helped by taking antibiotics. Overuse of antibiotics leads to the development of resistant bacteria, which can make subsequent infections harder to treat.

Billing Questions

Our office uses a separate billing company to coordinate billing issues with your insurance company. If you have a question about a bill received from Uptown Physicians Group, also known as David M. Lee, MD, PA, you can send questions through your portal account.

David M. Lee, M.D

Dr. David M. Lee grew up in Princeton, TX. He attended the University of North Texas where he graduated summa cum laude. He graduated from the University of Texas Medical Branch in Galveston with honors. His residency in Internal Medicine was at Baylor University Medical Center in Dallas, where he remains an active member of the medical staff.

Donald A. Graneto, M.D

Dr. Graneto works to keep Dallas healthy - one patient at a time - by creating individual health plans customized to each patient's unique needs. He grew up in Cleveland, Ohio, and graduated from The Ohio State University College of Medicine before moving to Dallas.

Eric Klappholz, NP-C

Eric Klappholz, ANP-C was born and raised in North Richland Hills, Texas. He attended Texas Woman's University where he graduated with honors (Sigma Theta Tau) and earned a Master's in Science as an Adult Nurse Practitioner.

Peter Triporo, MSN, APRN, ACNP-BC, AAHIVS

Peter Triporo MSN, APRN, ACNP-BC, AAHIVS, attended the University of Texas at Arlington, where he received his Masters of Nursing with Honors (Sigma Theta Tau and Phi Kappa Phi). He is board certified through the American Nurses Credentialing Center (ANCC) and licensed by the Texas Board of Nursing as an Adult Acute Care Nurse Practitioner.

Jason Vercher, PA-C

Jason Vercher, PA-C grew up in Pineville, Louisiana. He graduated Cum Laude from the University of Texas at Arlington with a degree in Biology. He completed his Master of Physician Assistant Studies at the University of Texas Health Science Center at San Antonio in 2016 where he was ranked in the top 5 of his class.

Vanessa Bludau, MSN, APRN, FNP-C

Vanessa Bludau, FNP-C was born and raised in Texas. She attended Texas Tech University where she received her Bachelor's of Science in Nursing with honors (Sigma Theta Tau). She graduated in 2012 with her Master's of Science as a Family Nurse Practitioner from Texas Woman's University with a GPA of 3.9.

William Anthony Hays, MD, ATC, LAT

Dr. Hays grew up in Iowa then relocated to Arkansas with his family while he was in high school. He later attended the University of Arkansas for his undergraduate studies and received a Bachelor of Science degree in Education with an emphasis in Athletic Training.
