After logging into the patient portal, you'll click the Visits button and select Appointments and Visits. Next to your appointment information, there will be a green eCheck-In button, taking you to the form. At the end of the form, you will electronically sign the document. After clicking the Submit button, your eCheck-In will be complete.

Dr. Haber trained in Internal Medicine at Maimonides Medical Center in Brooklyn, New York. After a brief stint as Director of Emergency Medical Services in the same institution, he went on to do two fellowships in Endocrinology. He studied Metabolism for two years at Johns Hopkins Medical School.
