myUHealth is an online secure tool provided by University Health to our patients to help them better manage their care and improve their overall healthcare experience. myUHealth can be accessed 24 hours a day, 7 days a week, 365 days a year from any device (computer, tablet, smartphone, etc.) with an internet connection.

If you have an activation code, you can use it to sign up.
Under the Health Insurance Portability and Accountability Act (HIPAA) of 1996, you have the right to access, inspect and/or obtain a copy of your protected health information (PHI), including: Name. Address. Important dates (birth date, admission date, discharge date, etc.) Medical record number. Social security number.
The first 10 pages of your medical record are free. A fee per page will be applied to any pages after that.
You can also view up to 20 pages of your medical records at University Hospital. Any additional pages must be requested through our Medical Records Department at the Robert B. Green campus.
Release of medical records requires a signature. Please download and fill out any of these necessary forms to obtain your medical records.
