How do I obtain a copy of a patient's medical records?

To obtain copies of health information, the hospital must have an authorization form; this form must be signed by the patient or his/her legal representative. A picture ID will be required. Records can be made available electronically, upon request.

Services: Medical records provides services to outpatients and inpatients, such as: copies of medical records, registering of live births, insurance requests, and coding of diagnoses. All medical records must be requested in-person at the Medical Records Desk by completing a form with signature.

How to get a copy of a medical record?

Below are the standard fees for producing a copy of your medical records: 1 Electronic records delivered in electronic format (email/CD) $6.50/per request 2 Electronic records delivered in paper $0.90 labor cost to create and deliver the portion of the record maintained electronically plus $0.05 per page for paper and toner 3 Paper records delivered in paper $0.12 per page 4 Postage and taxes will be applied where applicable

Where to send subpoenas to Christianacare?

Subpoenas must be mailed or delivered in person to Corporation Service Company (CSC), ChristianaCare’s registered agent, at 251 Little Falls Drive, Wilmington, DE 19808. Outside of Delaware, subpoenas should be sent to CSC in the state of the facility they are seeking to subpoena.
