Transferring Your Medical Records

Having your health history on hand makes it easier for us or your next clinical team to take better care of you. To transfer your medical records to Boynton or from Boynton to your next provider/clinic, complete this health information release form.

Immunizations, Vaccinations and Registration Holds

Minnesota law requires all college students to follow vaccination requirements. Failing to follow these requirements can result in a registration hold.

Health Insurance Coverage

Boynton Health requires patients to have health insurance coverage. We accept most insurance plans. Check with your health insurance provider to see if Boynton is in their provider network.

Student Health Advisory Committee

The Student Health Advisory Committee advises Boynton on how to improve the health of the university community.

Absence Notes

Under the University of Minnesota's Make-up Work for Legitimate Absence Policy, Boynton Health does not provide absence notes. The policy explains what you need to do if you miss class or coursework. Learn more about the absence note policy.

Who Can Be Tested at Boynton

COVID-19 testing from Boynton is available to all University of Minnesota students, faculty, and staff systemwide.

About COVID-19 Testing Appointments

Boynton Health offers COVID-19 testing by appointment only to University of Minnesota students, faculty, and staff systemwide. Testing is done within a secured area of the Recreation and Wellness Center (RecWell). Limited hours available for testing at the St. Paul Clinic in Coffey Hall as well.

Eye Clinic Safety Precautions

We are committed to our patients’ safety and well-being during this COVID-19 pandemic. These precautions provide the safest possible environment for our patients and staff:

Patient Safety Precautions

We screen all patients for COVID-19 symptoms and check their temperatures before entering our clinic. People with symptoms have to reschedule their appointments.

No-cost Eye Exams for Students

Students who have paid the Student Services Fee may receive one comprehensive eye exam (excludes exam for contact lenses) per year at Boynton at no out-of-pocket cost.

Vision Services

We evaluate your internal and external eye health, check for glaucoma, cataracts and eye problems associated with disease like diabetes and high blood pressure. Annual eye exams are recommended.
