How do I find a doctor in mile square?
If you're a regular Mile Square patient who needs care quickly, but it's not an emergency, please call 312.996.2000 and ask for the doctor on call. This health center also includes electronic dispensation of medications and on-site Certified Application Counselors (CACs) available to educate and enroll patients for health insurance.

Monday through Friday: 8 am - 7 pm Saturday, Sunday, and Holidays: 10 am - 5:30 pm Note: hours may change due to volume

Option 1 – During your next visit to UI Health
Our service desk will ask for your e-mail address and a one-time-use security question to start your account.
Option 2 – Self-enrollment from anywhere
If you have your Medical Record Number (MRN) or if we have your e-mail address on file, you can sign up for the portal remotely. Your MRN will be printed on your visit summary and discharge paperwork. You must be over age 18 to enroll.

About the Cures Act
The 21st Century Cures Act is a federal mandate that is designed to give patients more access to their medical information. As of March 31, 2021, UI Health is providing test results and provider notes (with a few exceptions) will be released via the MyChart patient portal as soon as they are available.

to Make An Appointment
- Please call 312-996-2000 to make an appointment. Please arrive 15 minutes before your appointment to register. If you are a new patient, please arrive 30 minutes before your scheduled appointment time. Should you need to cancel, please let us know at least 24 hours in advance.
Walk-In Visits
- Same-day appointments are available on a limited, first-come-first-served basis, and wait times will vary. Our Urgent Care Center also is available – however, due to demand, there may be times that the Center is no longer accepting new patients.
Cost
- Mile Square Health Center provides medical care to everyone, regardless of their ability to pay. Mile Square accepts a broad range of payment and insurance plans. Based on their income and family size, many of our patients qualify for low-cost services.
What to Bring
- When you come to Mile Square, with an appointment or for convenience care, please bring the following: 1. An insurance card for yourself and anyone else in your family who is receiving care (if you have one) 2. A list of all medications you currently are taking, including vitamins and dietary supplements 3. If your children are receiving care, please bring their immunization cards
