Who can use UHS?

UHS is excited to offer you the enhanced MyChart patient portal experience to better manage your health! Please note, your UHS medical information for approximately the past 3 years only will be in your MyChart account. If you have questions about your prior medical history, please talk with your provider.

What is a patient portal?

Non-HarvardKey Holders: Log in with your Username assigned to you by HUHS Health Information Services/Medical Records. Patients who do not have a portal account can create one by sending an email request to mrecords@huhs.harvard.edu and one of our HIS team members will reach out to you directly to provide assistance.

What is a physician portal?

Login to UW–Madison University Health Services. Login with your UW–Madison NetID. Choose MyUHS username only if you do not have a NetID. Email us at UHS@uhs.wisc.edu if you are unable to log in.

Are you a student coming to campus?

Please email UHS-HIM-Registration. Please include your UMID number, date of birth and a local address.

To restore your account

If you forgot your username or password, go to MyUofMHealth.org and click the Forgot Username? or Forgot Password? links. To submit a request, you will need your medical record number (MRN), which you can get by calling the Patient Portal Help Desk at 734-615-0872 (7:30 am - 4 pm).

There's an app for that!

In your app store, select the " MyChart by Epic " app, then choose " MyUofMHealth ." Once you have the app, sign in with your patient portal log-on and password. Please note that documents such as records and letters cannot be viewed via the app.

MyChart is a new patient portal that keeps all of your health information together in one place. Bill Pay and CareLink Re-Enrollment will continue to serve you.

Bill Pay Online

Bill Pay Online is for you to pay your statement for dates of service prior to July 11, 2020, if you have visits after July 11, 2020 please refer to MyChart. Call Customer Service at 210-358-9500 or toll free at 877-766-5533 Monday – Friday 8:30 a.m. - 5 p.m. or email questions at UHS.PBS@uhs-sa.com.
