This Task has been designed for use by Student. Use the Student Health System to manage appointments with Student Health, complete forms, including the medical history form, and send secure messages to Student Health. There are no new Announcements.
Both forms are submitted within the UD Health Portal. To log in, you will need to enter either your UDelNetID (username) and Password or your UD ID (usually a nine digit number beginning with 7) and your Pin.
