How to log into online patient portal?

You must specify if you would like to grant the adult proxy full information access or limited information (immunizations, allergies and secure messaging only) access to your portal information. Are you a student coming to campus?

Please email UHS-HIM-Registration. Please include your UMID number, date of birth and a local address.

To restore your account

If you forgot your username or password, go to MyUofMHealth.org and click the Forgot Username? or Forgot Password? links. To submit a request, you will need your medical record number (MRN), which you can get by calling the Patient Portal Help Desk at 734-615-0872 (7:30 am - 4 pm).

There's an app for that!

In your app store, select the " MyChart by Epic " app, then choose " MyUofMHealth ." Once you have the app, sign in with your patient portal log-on and password. Please note that documents such as records and letters cannot be viewed via the app.

Can my doctor outside of U of M access my records?

At the University of Michigan, we participate in a state-wide health information exchange (HIE) that allows other doctors and hospitals outside of U of M to access information that we’ve collected about patients. Also, there are other ways that patient information is shared electronically with health care providers outside of U of M.

May I have copies of my medical records?

Can you fax me my information?

We are committed to protecting your privacy and do not fax PHI (protected health information) per health system policy. Additionally, your medical record may contain sensitive information protected under federal and state regulations (including information about psychiatric, drug and/or alcohol abuse, HIV/AIDS, and or sexual abuse.

Is there a charge for my records?

There are no fees for sending your records to another physician or health care facility for the continuation of your care. Medical records sent to you directly through your MyUofMHealth Patient Portal also do not have any fees.
