Following the 2004 and 2005 hurrica nes that devastated the Treasure Coast region of eastern Florida, TCCH's Governing Board of Directors took the bold step of a planned expansion to improve access to care throughout the county, with a corresponding name change to Treasure Coast Community Health.
Additional services that remove barriers to care, like prescription assistance, translation services, social service referrals for food and housing, Medicaid eligibility/ enrollment, and assistance understanding the Federal Health Insurance Marketplace (through Certified Application Counselors), are offered by our Health Navigators.
Today, TCCH provides comprehensive primary and preventative care to over 23,000 individuals annually between its seven clinical IRC locations. TCCH uses a primary care delivery model, called a Patient Centered Medical Home, to provide integrated and comprehensive medical, dental, and mental health/substance abuse services to all people, whether insured, uninsured, or under insured, offering a sliding fee scale discount program based on income and family size.
