New Patient Forms and Insurance Information

Prior to their first appointment, families are asked to complete a Patient Registration form for each child. If you’d like to transfer your child’s records to Touchpoint Pediatrics from another practice, download our Transfer of Records form, complete it, and send it to your previous pediatrician – we’ll take care of the rest.
