How do I request patient access to a Southwell/Tift facility?

STEP 2 You will receive an email with a link to the Patient Portal (hospital patients will receive email upon discharge). STEP 3 Check your email and use the link to launch the on ine Patient Portal. Click on either the hospital or clinic link. STEP 4 You will be prompted to enter your date of birth to confirm your identity.

Tift Regional Campus

We are adding a brand new $150 million patient tower to our flagship Tift Regional Medical Center that will include new private patient rooms and a new Emergency Department set to open in 2021. Call our Concierge line at 229-353-CARE for help with your visit.

Cook Campus

The brand new $50 million Cook Medical Plaza is located right off exit 37 on I-75. Facilities include a hospital with state-of-the art operating rooms, a 95-bed skilled nursing center, and a medical office building for physician clinics. The new Cook Medical Plaza opened Fall 2019.

Tift West Campus

The West Campus, conveniently located right off exit 64 on I-75, is home to both the Southwell Medical Clinic and the brand new $22 million Musculoskeletal Center that opened in 2018. The West Campus has over 45 physicians serving in a wide variety of specialty clinics.
