How do I connect my children to the HPA portal?

TO USE THE PATIENT PORTAL... Please email hpa@huntsvillepediatrics.com with your children's names and birth dates. HPA will then send an invite to the portal. When setting up your portal account use a parent or guardians information for the log in process, your children's information will then be linked to your account.

1) At your next visit, ask a person at the check-out or check-in desk to print you off a pin # for patient portal. When they print this off, it will list a website that you can go to, in order to setup the account. 2) From your cell phone, go to your app store (either iPhone or Android) and search for Patient Portal.

Preparation for your Appointment

We ask that you please arrive 15 minutes prior to your appointment, this gives time to fill out all necessary forms.

New Patients

Welcome to Thomasville Pediatrics, we look forward to being a part of your child’s care. In order to better serve you and your child, we ask that you provide previous medical records, including immunization history prior to your first visit if possible. You can use the Medical Release Form to obtain this information.

Well-Child Exams and Immunization Schedules

Well-Child exams are an important part of maintaining your child’s health. During these visits we will discuss worries, concerns, or any questions you may have as well as sleep patterns, diet, school, and behavior. We do hearing/vision evaluations and discuss developmental milestones depending on the age.

Sports, Camp, College Physical

Older Children and teenagers often require forms for sports, camp, or college. Please bring specific forms necessary for your child to the visit. If your child has had a physical within the past year, we will be able to fill out the form from that physical.
