What is MyTorranceMemorial? The portal is your personal health connection to portions of your medical record. Here you can securely access information about your health at any time – conveniently view test results, print immunization records, view medication lists and more. What is the portal NOT intended for?
The Aspen Valley Hospital Patient Portal gives you access to your dashboard, which allows you to view current medications, your latest lab results, your medical records and any other patient specific updates. This gives YOU the control of obtaining your medical history at any time.
To access records through the patient portal, patient (or the designated representative) must provide a written and signed authorization. Patient PHI will be stored on the electronic health record regardless of whether you access the patient portal

Please download the form, fill out and return to the Aspen Valley Primary Care clinic prior to your upcoming appointment. The form can be taken to either Primary Care location in Aspen or Basalt, or faxed to the Primary Care clinic at 970-927-3915.
