Appointments & Referrals

Patients with ongoing medical issues such as Diabetes, High Blood Pressure, Cholesterol, Thyroid, etc. are asked to schedule their follow-up appointments when they are in the office.

When You Visit

When you come to our office you will be asked to bring a photo ID and your insurance card with any co-pay that you are responsible for.

Prescription Policies and Information

Prescription refills will be handled at office visits, and enough medicine will be prescribed to last at least until the patient's next appointment.

Please note, the office will be closed August 26th and 27th while we move to our new location at 400 Altair Parkway Suite 3100.
