If you feel you need to be seen by a provider, please call (828) 258-8681. We will direct you to the appropriate office location, or, we may recommend a Telehealth visit.

Telehealth Appointments – What to Expect

If you have a Smartphone, you will receive a text at your scheduled appointment time. The text will come from an unknown number. Please tap the link in the text message within 10 minutes of your appointment time. If you miss the text or do not tap the link in time, your provider will telephone you.

How do I sign up for the Patient Portal?

If you are a new patient, you automatically get a Portal account with a temporary password. Temporary passwords are only available for 48 hours. If this password expires you will need to get a new password from your Upstate Family Health Center office or call Portal Support at 315-624-9470

What if I forgot my username or password?

You will need to reset your username or password. There are two ways to update these Portal credentials:

Can I schedule an appointment on the Portal?

Yes. You are encouraged to use the Portal for appointment scheduling. Once you log into your Portal account, navigate to the "Appointments" section of the dropdown menu and submit your "New Appointment Request."

Can I request a prescription refill on the Portal?

Yes. You can request refills on the Patient Portal by clicking "Refill Requests" in the "Messages" section of the dropdown menu on the Portal homepage.

Can I ask my provider a question through the Portal?

Yes. The Patient Portal is a fast, simple and secure way to contact your Care Team with non-urgent medical questions. Click "Ask the Practice Team" to get routine matters addressed qucikly whether you're at home or on the go.

Can I get my lab results on the Portal?

Yes. You can view your lab results after your clinician publishes them. If your lab results are not posted on the Portal, they may not be final. You will receive an email when your results are ready for you to view.

Your patient account will allow you to securely and conveniently gain access to your important medical information to better manage your and your family’s health. Your medical information is available to you on this web-site and is secure, just as online banking and online stock accounts are secured via the Internet.
