We have two convenient locations for you to choose from. Our providers are available at both locations on designated days.

Patient Registration

New Patients On your first visit, we will ask you to complete forms that will provide us with personal and medical information vital to your care. New patient registration forms are available for download and completion prior to your visit.

Patient Portal

The Heart and Vascular Institute offers many services through our Patient Portal. Not only do you have the ability to review and update important elements of your medical record, you can also directly communicate with us regarding appointments, prescriptions, and test results.

This is my first visit with a cardiologist. What information do I need to have available?

Answer: If you have been seen by a previous cardiologist, please bring their last office note, EKG, lab work and any diagnostic results or procedure results with you. If you are unable to physically bring these records, you must request a release of information (ROI) from them in order for us to obtain the records on your behalf.

What do I need to bring to my upcoming cardiology appointment?

Answer: It is very important for you to bring your medication list and/or medication bottles with you to the appointment so that we have an accurate list within your medical record.

I was recently in the hospital, what information should I bring with me?

Answer: The hospital discharge summary is helpful, however may not contain detailed information from your visit. If you were seen in an Orlando Health hospital, we will have access to your records. Otherwise, please ensure you bring your records with you, including medication adjustments and any recent heart testing or heart procedure results.

How can I make the most of my appointment?

Answer: Come with a prepared list of questions and concerns to discuss with your cardiologist. Utilize the communication tool to organize your thoughts and requests. Bring a notebook with you so that you can keep a record of the things that were discussed. This will allow you to reference it later after your appointment.

What can I expect when I arrive to check in for my appointment?

Answer: Please plan to arrive 10-15 minutes earlier than your scheduled appointment as you will be required to update contact and insurance information and sign consents for treatment. How do I request a medication refill?

Answer: The quickest way to communicate your refill requests is through MyChart. You may also contact our prescription refill voicemail line at (407) 650-1397. Please allow 72 hours for all prescription refills.

How do I reach a member of my personalized clinical care team?

Answer: MyChart is the quickest and easiest way to reach a member of your personalized clinical care team. This option is available for non-urgent questions and concerns, making it easy to send direct messages to the registered nurse and medical assistant. They will respond within 48 hours.
