Patients 18 years and over may go online and Request Activation Code by completing the MyChart Access Request form.
First call Medical Records at Texas Children's Hospital at 832-824-1600. They will get you the forms you need to allow your medical record to be sent to your new health care provider. Medical Records will either mail or fax the forms to you. Note, you might have to wait up to two weeks to get the forms in the mail.
