How do I refer my child to the endocrine and Diabetes Center?

To refer your pediatric patient to the Endocrine and Diabetes Care Center, please complete the following steps. If you are unable to submit supporting medical records and lab results via our online form, please fax your referral order and supporting clinical information to 832-824-7333.

What is the diabetes and endocrine care center?

For established diabetic patients and/or referred inpatients with diabetes, the Diabetes and Endocrine Care Center provides refresher education, as well as support groups. The clinic team consists of a nurse educator, dietitian, physician and social worker. Patients are seen through referral only.
