Your medical records may be given to you for personal access and/or transferred to other healthcare facilities. To request your medical records, simply ask admissions staff or your nurse. You’ll be given a form to review and sign. For added convenience, you may request your records online through use of our patient portal or here: 1 Medical Release Form (English) 2 Medical Release Form (Spanish)
If home care services are not provided as planned, please call 208.354.2383 and let the discharge planner know so that further arrangements can be made. While at home, if you feel concerned about any medical condition, contact your physician immediately. Our Emergency Department is open 24 hours every day.
Our discharge planner will provide you with information about home health care, medical equipment, skilled nursing facilities, transportation, Meals on Wheels, and other community resources. Assistance with questions regarding Medicare, Medicaid, Social Security, State Disability and Workers’ Compensation can also be provided.
Visiting hours are dependent on the patient’s condition , and visits may be restricted at the discretion of the charge nurse. Make sure you’re healthy before planning a patient visit. If you’re experiencing nausea, coughing, sneezing, unexplained rash, or other symptoms of illness, please do not enter patient areas.
A responsible adult needs to be available to provide transportation to your home. If this is not possible, Teton Valley Hospital will make every effort to help you coordinate your transportation. If you’re having trouble finding transportation, please tell your nurse.
