What is the patient portal?

The Patient Portal is a secure, confidential and easy-to-use website that gives you and your family 24-hour access to your medical records. It uses the latest encryption technology to deliver secure communication between you and your providers. If you are not yet registered, please see the Patient Portal Registration Instructions to the left.

How do I update my profile on the patient portal?

You have an account. Now just update your profile and click “update profile.” Send and view messages to/from your care team. View past and upcoming appointments. The Patient Portal is a secure, confidential and easy-to-use website that gives you and your family 24-hour access to your medical records.

How do I take advantage of online patient portal services?

Take advantage of the online services offered by our practice with the assurance that all of your information is encrypted and stored securely. After you create your account, you will receive an e-mail from us that includes your user ID and a link back to our patient portal.

How to request a copy of my medical records?

For a copy of your medical records: 1 Please visit the link below to fill out your record request. You will need your ID to submit with your request. 2 After your request has been received, our medical records provider (Sharecare Health Data Services), will process within 24 to 48 hours and generate an invoice (if over 20 pages). 3 Records will be sent once payment is received. You may choose either mail or email as delivery options. There is no fee for records sent directly to another healthcare provider. You may choose mail or fax as the delivery options (not both). 4 For questions about medical record requests contact (866) 641-4778. 5 Please note, it may take up to 7 business days to receive your medical records.
