How do I enroll in the online portal?

In person: Provide your email address at registration and we'll email you an invitation to join. Online: Enroll in the portal online at any time after registration. When you enroll, enter your Medical Record Number (MRN) found on our hospital wristband or on documentation provided at discharge.

Patient Forms

Please call 951-677-4748 Monday through Friday from 9 a.m. to 4:30 p.m. to schedule. We require a 24-hour notice for cancellations. Please arrive 15 minutes prior to your scheduled appointment.

Prescription Refills

To refill a current prescription, please call your pharmacy and request a refill. If you do not have any refills left on your prescripton, your pharmacist will contact us for authorization. Please allow one working day for refills.

Lab Results

You should receive notification of lab results by phone or mail. If your results require immediate attention, we will contact you by phone. You should receive notification of a normal Pap Smear within 2 weeks of your visit. You will be contacted by phone if your Pap Smear is abnormal.

Medical Questions and Concerns

We have trained personnel with many years of experience who triage calls everyday from 9:00 a.m. to 4 p.m. They are available to address non-emergency questions. All calls prior to 4 p.m. will be returned the same day. Calls received after 4 p.m. will be returned the next business day.

Obstetric Services

In addition to our general obstetrical services, we also provide care for high risk pregnancies, including women with pre-existing medical conditions, multiple births or abnormal pregnancies that carry an increased risk of complications.

General Obstetrical Care

Obstetric Services We provide comprehensive obstetrical care from preconception to postpartum. Our experienced doctors provide general services to guide women through each and every step of their pregnancy to ensure proper growth and development, as well as a successful delivery.

Infertility

The standard definition of infertility is the inability to conceive after 12 months of unprotected sexual intercourse. Today, this has been modified to take age into account, and now women over 35 may be considered infertile if they have failed to conceive after trying for six months.

We now offer the ability to securely connect your health record data to approved third-party applications. Visit the Google play or Apple app store to download third-party applications.
